Help — Using the online service submission form

The online submission form is a simple and effective method for placing your submissions with Cerberus.

To begin your submission, please fill out the details section, any field marked with an (*) is required, however we do ask if you can fill the majority of the fields in this section it is preferrable.


The Notes field can be used to supply Cerberus with any relevant information, you do not need to fill this field in, unless it is applicable to your submission.

The type and submission categories can be selected individually or in any combination required.




The Pathology use only section of the form is required only for Pathology based submissions, please leave this area blank if you are not using Cerberus Pathology services.




The submission section of the form uses a combination of text entry fields and drop down menus. It is important that you fill out every field in this area.

If you need to specify a different test to those provided in the Test(s) Required drop down menu, please choose 'Other'.

On selecting the 'Other' option, the text entry field to the right of the drop down will become active.




Once you have completed a submisiion entry, click the ADD button to create a new submission row. Our form will accomodate thirty sets of fields. In the event that you require more, please repeat the submission process.




At the base of the form is are Submit and Reset buttons.

Click the Submit form to send your form to Cerberus, on submission, you will be presented with an onscreen summary of your submission that you can print out as a record.

Click the reset button to clear all the fields in your form.
